Job Description:

Springfield Medical Care Systems (SMCS), a newly independent FQHC comprised of multiple PCMH practices delivering team-based care in Southern VT, is seeking an experienced nursing professional who is passionate about community-based care to join our leadership team. Coming on board at this transformative time as we build our new organization, the Director of Nursing will set standards for nursing practice, support and develop all levels of staff in models of integrated, team-based care, lead our community health and care coordination teams, and strengthen our efforts to achieve the Quadruple Aim and our mission of high-quality person-centered care. You will:

  • Play a leading role in the recruitment, development and support of a dynamic and mulit-level clinical nursing staff; support the efficient deployment of clinical support staff, ensuring the right skill mix to promote the highest level of care
  • Develop a positive culture to promote continuous growth and staff development, establish best practices and the education and support systems to sustain them
  • With the executive leadership, medical and nursing staff and key managers, participate in the development of health care policies, procedures and protocols while ensuring their consistency with applicable laws, regulations, program expectations, and professional standards.
  • Oversee the development and implement of care coordination in accordance with FQHC, PCMH, Blueprint for Health and ACO standards, values and models
  • Provide supervision and mentorship to Care Coordinators and CHT staff, including CHWs, CDEs and RDs, and MAT staff
  • Be a champion for quality improvement and innovation, encourage and lead QI projects, evaluate workflows, develop innovations and participate in data reviews and learning collaboratives
  • Establish and maintain relationships with community service providers; serve as a resource to staff on how to coordinate client access to services.
  • As a member of executive leadership, engage regularly with the Board of Directors, participate in executive level decision-making, strategic planning, the development of business initiatives, and advance of the overall mission and goals of our organization.
  • Develop and promote partnerships with providers, payers, community organizations, the ACO, employers and other stakeholders to achieve quality and population health strategic goals and programs. Requirements You'd be a good fit if you have:
    • Bachelor's Degree or Master's Degree in healthcare or related field.
    • Minimum five years of professional nursing experience in a medical practice setting or hospital. Primary care of FQHC experience is highly desired.
    • Minimum five years of leadership experience demonstrating managerial and operational expertise and competence including experience in primary care medical practice leadership, understanding protocols, processes, and procedures.
    • Experience with and an unwavering commitment to quality programs and care, cultural competence, community/public health, and data-driven program evaluation.
    • Knowledge of strategic planning, resource allocation, productivity and performance measurement.
    • Knowledge and understanding of HRSA Health Center Program (330 Grant) requirements. Including the following:
    • Familiarity with Quadruple Aim Principles, Value Based Care model, Uniform Data System (UDS) and Healthcare Effectiveness Data and Information Set (HEDIS)
    • Familiarity with Patient Centered Medical Home model, concepts, competencies, and criteria, Care Coordination and Care Management programs/services
    • Excellent communication skills; verbal and written; conflict management and resolution abilities
    • Demonstrated basic knowledge of safety regulations such as OSHA, Emergency & Crisis Management, de-escalation of emergency situations, etc.
    • An awareness of and sensitivity to the cultural diversity of the population served;
    • Ability to maintain a non-judgmental disposition and communicate with a diverse population; a desire and ability to work closely with a low-income population;
    • An appreciation and passion for life in rural Vermont, our long winters and sweet summers! Nice to have:
