STATUS: ExemptREPORTS TO: Regional Director Home HealthSUPERVISES:
Clinical Team Members, RNs, LPNs, HHAsENTITY: OVNA $15,000 Sign-On
Bonus (based on FTE)POSITION SUMMARY:The Manager of Clinical
Practice (RN) directs and supervises the office operation and will
be available for OASIS Assessments in the field. Guides team in
planning and providing individualized care and comprehensive
services to home care patients. Assists in clinics and other agency
programs as assigned by the Regional Director. The Manager of
Clinical Practice (RN) is expected to meet established productivity
standards as well as a rotation of manager on-call

FUNCTIONS:Monitors assigned workflow tasks in HCHBProvides clinical
direction to field staff as neededCodes commercial OASISOrganizes
educational programs to address identified needsReviews clinical
records for completenessDemonstrates prioritization, organization
and appropriate management of timeKeeps current with all regulatory
requirements affecting Home Health servicesOversees Medical Supply
ordersCollaborate with supervisory staff to promote accuracy and
effectiveness in documentationParticipate and lead case conferences
and staff meetingsEducates staff on deficiencies found through
audits as neededEvaluate clinical competency of nursing
staffParticipates in the orientation of new team membersManage
Telehealth programsPerforms Blood Pressure clinics, as
requested.Assists with clinical oversight at the Overlook IL and
AFL as neededProvides direct skilled nursing care as per physician
orders in the field with patient visits as neededOn-Call triage
coverage as needed.Documents in medical record and prepares reports
as needed.Educates patient and families in self-care and health
managementCollaborates with the physician and other home health
care providers, within and outside agency to enhance patient
care.Adheres to policies and proceduresParticipates in agency
quality management and safety activitiesServe as coverage role
during staffing challengesAny additional responsibilities as
requested by the Regional Director Home Health and Executive
Director Home Health and HospiceSUPERVISORY RESPONSIBILITIESActs as
Home Care Home Base (EMR) resource with extended access and
rolesCompletes RNs, LPNs and home health aide supervisions and
orientations as neededParticipates in training new clinicians on
HCHB and homecare regulationsCo-signs orders and provides oversight
of care for RNs and LPNsQUALIFICATIONS:Graduate of an accredited
school of nursing required, BSN preferred, MSN strongly preferred
or Bachelor's degree in another discipline with home care
experienceMinimum 3 years' experience in a managerial roleMinimum 5
years' experience in a patient care settingCurrent license to
practice as a Registered Nurse in the state of Massachusetts
requiredCurrent CPR requiredCurrent MA Driver's License and
reliable transportation requiredDemonstrated collection and
supervisory skillsExcellent organizational and interpersonal
skillsComputer proficiencyExcellent verbal and written
communication skillsAbility to read, write and speak the English
languageMaintain required vaccinationsPHYSICAL REQUIREMENTS/WORKING
CONDITIONS:Primary work will be performed in doors in an office
setting, may be exposed to the elements (heat, cold, rain, snow);
walking and standing frequently; occasional lifting of up to 25
pounds. Completing visits in the field as necessary requires
