STATUS: ExemptREPORTS TO: Executive Director of Home Health and
HospiceSUPERVISES: All Hospice Team Members, Direct and Indirect
reportsDEPARTMENT: Hospice ServicesENTITY: OVNA**$15,000 Sign-On
Bonus (based on FTE)POSITION SUMMARY:The Director of Hospice
Services is responsible for all aspects of Hospice services for The
Overlook. They are charged with growing the organization across via
effective partnerships, marketing and excellence in care while
maintaining a careful eye towards maintaining the highest standards
of compliance with the State and Federal regulations and promoting
excellent relationships with all primary constituencies-patients
and families, Team Members, referral partners, and the communities
we serve.The Director of Hospice Services develops short and
long-term hospice service goals and plans ensuring alignment with
broader Overlook priorities, plans and goals. Utilizes metrics and
organization vision to lead and direct strategic practices and
standards. Develops and implements Hospice services practice
standards and guidelines. Partners across The Overlook continuum to
ensure maximum opportunities to provide our services.PRINCIPLE
DUTIES/ESSENTIAL FUNCTIONS:OVNA Leadership Team MemberServes as a
member of the OVNA Leadership team providing strategic direction on
Hospice related matters. Works effectively as a team member in a
dynamic executive environment and promotes teamwork within and
across the organization by maintaining positive relationships and
utilizing effective problem-solving skills.Financial
ManagementDrives the development of annual goals and the budget and
is responsible for meeting those goalsReviews financial indicators
and implements action and modifications when warranted. Maintains
clear expectations of accountability with direct reports.
Communicates with managers to resolve various issues related to
staffing, utilization of Hospice services, equipment and supplies,
and interdepartmental processes and hand-offs.Prepares update
reports for monthly Board of Director meetings.Sales and
MarketingDevelops and implements plans to grow the reach of the
Overlook Hospice organization.Develops and maintain relationships
with referral partners.Works with the Director of Business
Development and Marketing and team of community liaisons to
position Overlook Hospice for Growth.Operations ManagementEnsures
the Hospice organization operates efficiently and effectively,
while maximizing profitability and growth. Ensures improved
operational integration of home health, hospice and palliative
services with system services. Recommends changes to administrative
policies to achieve strategic objectives.Directs and oversees the
development of the organization's operational policies, program
objectives, and initiatives.Implements and makes recommendations
for changes to policies, branch goals/objectives, and work
processes annually.Identifies common opportunities for
standardizing processes to achieve a consistent experience for all
served. Develops strategies to ensure premier patient satisfaction.
Leads development of standards to facilitate effective utilization
of electronic health records. Assists leaders in administering,
directing and coordinating the activities of the hospice staff to
achieve strategic objectives.Ensure the appropriate scope of the
Quality Assurance and Performance Improvement Program as required
by program regulations.Performs patient visits as/if
necessary.Other roles, responsibilities and duties as assigned by
the Executive Director of Home Health and Hospice.Ethics and
ComplianceRegularly displays and encourages ethical behavior in the
areas of business ethics, legal compliance, confidentiality,
conflicts of interest, vendor relationships and the reporting of
questionable activities. Serves as a role model and mentor to
operation leaders.Maintains knowledge and achieves compliance with
federal, state and local regulatory requirements, licensure,
accreditation requirements, record procedures and internal
controls. Regularly informs and updates staff on organizational and
industry issues. Coordinates the provision of hospice care
activities to ensure patient care is provided in accordance with
regulatory, statutory and legal standards. Reviews pending
legislation on local, state and national levels for impact on
health care; and when appropriate, enlists support to influence
effective legislation. Develops and implements policies and
protocols, which comply with all laws and regulations that are
required for compliance with the Conditions of Participation and
the reimbursement guidelines for each agency.Reviews complaints and
grievances per Hospice policy and makes oral/written reports to the
CEO/COO when appropriate.Adheres to the organization's Standard of
Conduct and all other policies and procedures.Reports any violation
of the organization's federal or state laws and regulations.Quality
Care and Patient SatisfactionOversees Hospice operations to ensure
compliance with established corporate objectives and the delivery
of optimal hospice care services. Takes administrative calls as
assigned, solving problems and making appropriate policy
interpretations to ensure quality care to patients.Monitors
progress toward hospice patient satisfaction goals and recommends
and implements corrective actions as necessary. Establishes and
implements key quality metrics to evaluate the quality of patient
care. Participates in ongoing continuous quality improvement
education and trainingEnsures that the continuity and quality of
care for patients and families for all components of the program
are well coordinated and monitored.Maintains lines of
communications between the team members and patients/families,
physicians, other health care providers and other community
agencies.Assures that clinical documentation accurately reflects
both the care delivered and patient condition and participates in
the development of the plan of care for
patients/families.Periodically attends care conferences with the
patients and/or families and the interdisciplinary teams to ensure
high levels of effectiveness.Ensure the proper provision of
education, information and/or training for patients/families and
team members for specific care or procedures performed in home
care.Performs joint patient visits with team members as
appropriate.Responds to inquiries or situations involving
patients/families, team members or supporting professionals as
needed.People ManagementEnsures the engagement of Team Members
across the organization through the use of The Overlook's Guiding
Principles and Culture of Excellence. Ensures HR policies are
administered consistently across function. Organizes hospice
service areas through appropriate structure and delegation of
functions. Identifies succession planning needs.Promotes team
building and collaboration. Motivates, encourages and empowers
staff to achieve personal, organizational and team goals.
Recognizes accomplishment. Encourages and provides opportunity for
staff input and feedback regarding improvement opportunities.
Recommends and implements changes as needed to comply with
accrediting or performance standards.Partners with People & Culture
to ensure the hiring and retention of the Talent necessary to
achieve organizational goals.Team Member Training and
EducationEnsures the appropriate Team Member training and
development specific to agency and individual team member
needs.Ensures effective oversight and development of direct report
personnel and uses results to develop and implement appropriate
education and training courses to achieve and maintain compliance
with Medicare Hospice benefits.Attends and participates in
workshops, seminars, etc., to keep up to date on current
regulations and changes in the health care field and ensures other
do the same.QUALIFICATIONS:Bachelor level Registered Nurse in good
standing in the State of Massachusetts requiredMaster's degree or
equivalent years of education and experience preferredMinimum of 5
years serving in Hospice field - requiredMinimum of 3 years in
leadership/supervisory position requiredDemonstrated track record
of excellence in motivating, managing and leading teamsExcellent
organizational and interpersonal skillsKnowledge of:Best practices
for Hospice Services.Hospice regulations and procedures.Quality
