WHY THIS ROLE IS IMPORTANT TO US

The Advanced Practice Clinical Preceptor is a member care team who reports up to the identified site Clinical Director and works to develop strategies and maintain processes which support the orientation and training of novice and newly-hired clinical advanced practice staff. Primary responsibilities are precepting new and novice clinical staff to become independent and clinically competent members of the team; to ensure understanding of their role and function within the clinical team and to ensure staff are well-prepared to deliver CCA s clinically-driven, consumer-directed model of care. The Advanced Practice Clinical Preceptor also provides practice support to existing staff. The Advanced Practice Clinical Preceptor will utilize internally generated referrals requesting APC support, as well as the NEOs assigned panel, to help orient new staff.

WHAT YOU\'LL BE DOING

* Orient new staff to CCA s Model of Care; unit specific workflows; processes and role-based competencies

* Monitor and supervise new staff work processes and abilities to perform role-based competencies

* Develops and coordinates education experiences and clinical training for novice and newly hired staff

* Develops individual training plans based on clinician s needs in collaboration with the education team

* Participates as a member of the Multidisciplinary Precepting Team

* Provides leadership with respect to identification and dissemination of culturally and linguistically appropriate health education

* Evaluates and reinforces or teaches appropriate clinical assessment and examination techniques as appropriate

* Provides clinical consultation for condition /disease specific issues

* Reviews clinical documentation for accuracy, completeness and clarity

* Supports safe practice of the role and to the full extent of new hire s licensure, prescribing, education and training

* Develops and coordinates education experiences and clinical training for novice and newly hired staff

* Develops individual training plans based on clinician s needs in collaboration with Clinical Education team

* Works with the CCA Clinical Education & Staff Development Teams to standardize curriculums and training programs across the organization

* Familiarizes the newly hired clinician with available medical references and resources such as Up-to-Date, BCMG clinical guidelines and policies and procedures, Prescribing Letter etc.

* Participates in and uses evidence-based research to identify strategies for improving professional development activities, nursing practice and patient centered outcomes

* Ensures educational activities are congruent with CCA s mission, goals, values, priorities, and resources.

* Assists adult learners to identify both their learning needs and the most effective activities to meet those needs

* Provides an appropriate climate for learning and facilitates the adult learning process by developing, planning, and presenting educational activities

* Responds when remedial help is needed for individual clinicians and helps site manager develop corrective action plans

* Collaborates with others in the practice of fostering professional development at the institutional, local, regional and state level

* Participates in performance improvements projects as assigned

WHAT WE\'RE LOOKING FOR

* Master's Degree or equivalent experience

* Must be an Board Certified NP/PA licensed to practice in the state of Massachusetts

* Home care experience strongly preferred

* Must have 3 years recent active employment as an NP/PA

* 3 - 5 years relevant clinical, homecare, and case management experience desire

* Excellent interpersonal skills demonstrated ability to interact professionally with culturally and educationally diverse staff and patients

* Ability to develop plans of care that are individualized to the patient s condition or needs, reflecting current practice that is evidence-based, providing for continuity of care, and are family centered

* Creativity, flexibility, sound judgment, and the ability to work independently and take initiative

* Excellent time management, documentation and organizational skills

* Highly functioning computer/software skills, EMR systems, informatics and Microsoft Office programs.

* Strong communication and writing skills

* Demonstrated ability to work as an effective team member and leader in complex and fast-paced environment

* Demonstrated clinical competence in all aspects of primary caring nursing, care management, nursing assessment, care planning, and medical/surgical nursing.

* Strong desire to teach

* Must be able to make home visits in a community-based setting

* Must valid have driver s license and personal and reliable vehicle

* Current knowledge, competence, and certification as an NP/PA in the population foci consistent with teaching responsibilities

* Qualified through academic preparation/certification/licensure, as appropriate, to teach the subject assigned

* Leadership skills necessary to develop, create, and maintain educational clinical programs

* Proven skills, knowledge base, and judgment necessary for independent clinical decision making

* Excellent organizational, time management and problem-solving skills

* Ability to function effectively as part of a multi-disciplinary team

* Effective oral and written skill

* Strong interpersonal and customer relations skills

* Effective teaching skills

* Demonstrate attitudes and qualities consistent with the ethics of the health professions, including: Leadership skills in the areas of peer review, quality assurance, and community involvement while promoting the health of population groups

* Mentor, teach and support the student/employee

* Ability to provide accessibility to student for completion of projects/objectives

* Language skills not required but a plus
