STAR.Palliative Care/Heart Failure Advanced Practitioner Role Scope
The Nurse Practitioner or PA will work in partnership with the
interdisciplinary Palliative Care Team and The Center for Advanced
Heart Failure and Pulmonary Vascular Disease. The Nurse
Practitioner or PA will:Oversee a caseload of patients with
advanced heart failure in both the outpatient and inpatient
settings Maintain a process for continuity as patients present in
acute or ambulatory settingsDevelop and monitor palliative care
treatment plans, participate on heart failure team roundsInitiate
communication with patients and families around goals of care,
advance care planning, consideration of advanced cardiac
interventions and end-of-life issuesProvide symptom management
during illness exacerbations and at the end-of-lifeShift: Monday

Practitioner Requirements Palliative Medicine and/or Heart Failure
experience preferred Graduate of an accredited Nurse Practitioner
or PA-C program Licensed, or eligible to be licensed, as an
Advanced Practice Registered Nurse (APRN) or PA-C in the state of
Connecticut; federal DEA license requiredNational Board
