largest Program of All-Inclusive Care for the Elderly (PACE) in New
England and the fifth largest in the country. Called Summit
ElderCare, Fallon's PACE helps provide older adults and their
caregivers with a welcome alternative to nursing home care.
Participants in Summit ElderCare have access to comprehensive
medical services and social support at a Summit ElderCare site
while they keep the independence of living in their own homes and
communities.Position Summary:The Licensed Practical Nurse, under
the direction of the Clinical Manager, is responsible for the
quality of nursing care s/he provides to patients while adhering to
the standards of nursing practice.ResponsibilitiesPrimary job
responsibilities: As a member of the team, the LPN will also be
responsible for supporting duties and responsibilities assigned to
the Clinical Assistant and when needed so as to facilitate service
to patients in a timely manner. Provides direct and indirect care.
Identifies and records physical, social and psychological needs of
the patient and family. Initiates and completes nursing assessments
in collaboration with the Registered Nurse or provider. Establishes
priorities when working with patient in various clinics and assists
other staff members with their assignments. As necessary, assesses
and attends to patients needs promptly. Provides nursing
intervention according to the individual patient visit or
procedure. Provides pertinent data collected to Registered Nurse or
provider. Evaluates selected teaching and learning needs of patient
and families. Identifies patient situations that require immediate
action and responds accordingly. Participates with the professional
nurse in developing/revising the individualized plan of care.
Shares input from a patient and family with other health team
members during formulation of care plan. Collaborates with
professional nurse, physician or health associate in revising
patient care plan, utilizing current data or information from the
patient or family. Contributes information during patient care
conferences for evaluation and revision of teaching plan. Write
clear, concise nursing notes that reflect implementation and
evaluation of care plan. Identifies and interprets physiological
reactions of the patient. Utilize patient care equipment
appropriately and safely. Incorporates the proper requirements in
operating unit-specific equipment to provide safe patient care.
Incorporates nursing and station specific policies, procures and
protocols into the delivery of patient care. Adheres to
organization, department and unit-specific policies, procedures and
protocols. Completes the unit-specific competency requirements
satisfactorily and within the established timeframe(s).
Demonstrates knowledge and skills required to perform processes
involved in the delivery of age-appropriate patient care. Provide
care appropriate to the patient population served as directed by
the Registered Nurse or Provider. Completes and demonstrates
knowledge of mandatory programs/information. Information to be
completed includes: Fire Safety; Disaster Plan/Utility Failure;
Hazard Communication Standards (MSDS) sheets, if appropriate;
Infection Control/Universal Precautions; Equipment Failures (Safety
Medical Device Act); Security; Department's Office
