STATUS: Non-ExemptREPORTS TO: Hospice Clinical ManagerSUPERVISES:
N/ADEPARTMENT: HospiceCOMPANY: OVNA HospiceLocations: Greenfield,
Sunderland, Wendell. Based out of our Sunderland Mass OVNA
location.POSITION SUMMARY:The primary purpose of the position is to
assess, plan, organize, develop, and implement related social
services and community support strategies to our patients in
accordance with current federal and state regulations, the
organization's established policies and procedures, and as may be
directed by the Clinical Manager. To assure that the medically
related emotional and social needs of the patients are met /
maintained on an individual basis. This is a Per Diem Role.THE

performance or outcomesPRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:The
primary purpose of the position is to plan, organize, develop, and
implement comprehensive psychosocial care to patients and families
at the end of life in accordance with current federal, state, and
Medicare standards, guidelines and regulations, the organization's
established policies and procedures, and as may be directed by the
Administrator or Clinical Manager to assure that the medically
related emotional and social needs of the patient are
met/maintained on an individual basis.Provide medically related
social services to patients and families.Complete required
documentation including admission notes, weekly/monthly progress
notes, and IDG notes biweekly.Participate in IDG meetings and
attend regularly scheduled care plan meetings as
required.Coordinate family meetings as indicated.Advocate on behalf
of patients and families regarding issues as appropriate based on
patient Rights, including review of patient grievances.Be aware of
Medicare and Medicaid, and other financial and community assistance
programs available to the patient and provide education as
indicated.Provide guidance to team members regarding upholding
patient and family rights.Must comply with organizational vaccine
mandates and maintain required licensure and
certification.QUALIFICATIONS:Must possess a Master's Degree from an
approved school of Social WorkMust be licensed in MA as a LICSW or
work under the direct supervision of an LICSWMust have, as a
minimum, one year experience as a social worker in a skilled
nursing facility, hospice, hospital, or other related medical
facilityExperience with geriatric, oncology and/or hospice
preferredStrong verbal, written and interpersonal communication
skillsPossess valid State of Residence Driver's license and
reliable transportationPHYSICAL REQUIREMENTS/WORKING
CONDITIONS:Primary work will be performed in patient's homes,
nursing facilities, assisted living facilities and may be exposed
to the elements (heat, cold, rain, snow); walking and standing
frequently; occasional lifting of up to 25 pounds. Must be able to
travel to patient's location with reliable
