Sign-On Bonus eligible position.POSITION SUMMARY:The Home Health
Aide is responsible for providing personal care and related,
appropriate housekeeping activities to patients in their homes
under the guidance and supervision of the clinician responsible for
the patient, as well as working with other members of the team. Per
Diem. Based out of our Sunderland, Massachusetts VNA location.
Servicing Shelburne, Charlemont, and Heath communities. -THE

RESPONSIBILITIES:The following duties are normal for this position.

Assist patients with bathing,
toileting, dressing, shampooing, feeding, shaving, mouth care,
etc.Assist patients with moving in and out of bed, bath, wheelchair
or automobileAssist with simple exercise programs, application of
prostheses and braces, ambulation and transfer activities
(including use of assistive devices) and accompanying patient out
of doors.Application of supportive aides, such as slings and
elastic stockings.Temperature, pulse, respirationCatheter, cannula
care and ostomy assistAssist with medications normally
self-administered or remind patient as directed by the primary care
nurse. - Planning, shopping and preparing meals accordingly to
prescribed diet.Light housekeeping activities, such as changing bed
linens, laundry and cleaning as necessary to maintain a healthy
environment.Accompany patients to the physician or clinic, etc.,
when part of the care plan and approved by direct
supervisor.Maintain records of patient care, condition, progress or
problems to report to and discuss observations with supervisor or
case manager.Adhere to policies and procedures. -SUPERVISORY

High school diploma or equivalent -

Driver's License and reliable transportationSuccessful completion
of the Home Health Aide Competency Exam with a minimum - overall

Skills and Abilities:
Language Ability: - Ability to read,
write, speak and understand the English language. - Knowledge of
the structure and content of the English language including the
meaning of words, rules of composition, and grammar. - Ability to
effectively communicate with peers, supervisor, residents, clients,
and guests. Ability to read and interpret documents, such as weekly
schedule and patient plan of care. Mathematical Skills: - Ability
to add, subtract, multiply and divide in all units of measurement,
using whole numbers, common fractions and decimals. Cognitive
Demands: - Ability to apply common sense understanding to carry out
instructions provided in written, oral or diagram form. - Ability
to deal with problems involving several concrete variables in
standardized situations. - Able to swiftly change direction as
necessary.Computer Skills: Ability to use Microsoft Windows, email
and the Internet. - Ability to use hand held device / software for

ADAPTABILITY:
- Works primarily indoors in a varying climate controlled
