Job Description:
A company is looking for a Medication Therapy Management
Clinical Pharmacist.
Key Responsibilities
Evaluate medication profiles to optimize drug therapy and minimize
adverse reactions
Recommend alternative medications or modifications to therapy as
needed
Conduct comprehensive medication reviews with members and
collaborate with physicians
Required Qualifications
Bachelor's or Doctorate of Pharmacy degree
Minimum one year pharmacy residency training or equivalent
experience recommended
1-2 years of experience in a pharmacy or related healthcare
environment
Current, valid, and unrestricted state Pharmacy license
required
May need to obtain additional Pharmacy licensure in multiple states
within six months of employment
