Post Acute Care Opening in Albany, NYSeeking Post Acute Care Medical Director. Experience as a medical director in a skilled nursing facility preferred. However, clinicians who are board certified, with leadership experience are encouraged to apply. Full-time, Employed Monday - Friday daytime, flexible hours Well-developed infrastructure with extensive back-office support Must have experience as a Medical DirectorResponsibilities Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees Collaborate with leadership to review care coordination initiatives Develop intervention plans, and share ideas about network and risk management issues Work alongside pharmacists on medication management Implement an individualized plan of care for patients' goals Make recommendations on transitional care, DME, home health services, or outpatient services after short stay Timely response in chart completions and pharmacy recommendations Provide oversight and mentorship to our clinicians in the greater Albany, New York, areaCompensation & Benefits Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated range of $215,000 to $242,00 annually with no cap on productivity income potential.
