Join a traveling Physical Therapist team serving Home Health patients in Lake Katrine, NY, and surrounding communities including Kingston, New Paltz, Marlboro, Milton, Gardner, Wallkill, Ellenville, Napanoch, and Kerhonkson. Apply now and a recruiter will reach out with more details. Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active New York State PT license required prior to the start of the assignment. Current CPR (BLS) certification from AHA or ARC. Must have a valid driver’s license, personal vehicle, and vehicle insurance for travel to patient homes. Experience in home health preferred. Candidates with prior home health caseload management, documentation in electronic medical records, and working with interdisciplinary home care teams are preferred. Ability to complete patient evaluations, develop PT plan of care, submit progress notes per agency policy, and prepare written discharge summaries as required. May supervise PTA and home health aides when permitted by state regulations. Assignment Details Facility Type: Home Health. Position Status: Full Time. Coverage Area: Multiple locations within the Hudson Valley region including Kingston, New Paltz, Marlboro, Milton, Gardner, Wallkill, Ellenville, Napanoch, and Kerhonkson. Floating to support nearby locations may be required. Job Type: Travel Physical Therapist providing direct patient care in home settings. Clinical Responsibilities: Evaluate functional status, develop and implement plan of care, document visits and progress notes per policy, communicate with supervising nurse and physician, provide patient and caregiver education, and participate in team conferences.
