Job Description:
Albany Medical Center seeks a dedicated Clinical Nurse III Post
Transplant Coordinator to join our innovative team. This role
focuses on the care and management of post-transplant patients,
ensuring seamless coordination and high-quality support throughout
their recovery process. The coordinator will work closely with a
multidisciplinary team to optimize patient outcomes and enhance
their journey from pre-transplant preparation through to long-term
follow-up care. Monitor patient health status and recovery
post-transplant. Coordinate all aspects of patient care with
surgeons, physicians, and other healthcare professionals. Educate
patients and families about care processes, medications, and
lifestyle adjustments post-transplant. Implement treatment plans
and manage follow-up appointments to ensure comprehensive care.
Maintain accurate records and reports related to patient treatments
and outcomes. Strong clinical skills in post-transplant care.
Excellent communication and interpersonal skills. Ability to work
collaboratively in a multidisciplinary team. Strong organizational
and multitasking skills. Proficiency in electronic medical record
systems. Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Registered Nurse (RN) licensed to practice.
Certification in clinical transplant coordination preferred.
