Job Description:
Albany Medical Center is seeking a dedicated Clinical Nurse III
Post Transplant Coordinator to join our innovative healthcare team.
This role is pivotal in providing exceptional care and support to
post-transplant patients. The coordinator will manage patient care
in a comprehensive manner throughout the recovery process,
collaborate with multidisciplinary teams, and ensure adherence to
the highest standards of healthcare practice. Coordinate
comprehensive care plans for post-transplant patients. Monitor
patient health and respond to any complications. Educate patients
and their families about post-operative care and lifestyle
adjustments. Collaborate with transplant surgeons, physicians, and
other healthcare professionals to ensure continuity of care.
Collect, analyze, and report patient data to improve care
strategies and outcomes. Participate in research and development
activities related to transplant care. Strong clinical skills with
a focus on transplant care. Excellent communication and
interpersonal skills. Ability to work effectively in a
multidisciplinary team. High attention to detail and
problem-solving skills. Proficient in electronic health record
(EHR) systems. Medical Insurance Dental Insurance Vision Insurance
Life Insurance 401k Registered Nurse (RN) license in the state of
practice. Certification in Clinical Transplant Coordination
preferred.
