Nurse Practitioner (Primary Care) - Brattleboro, VT

Shift Scheduling: Full-Time Mon-Fri (8 AM - 5 PM)

About the Role

We are seeking a dedicated Nurse Practitioner (APRN) to join a collaborative outpatient primary care team. This role focuses on delivering high-quality, patient-centered care with an emphasis on prevention, wellness, and chronic disease management.

Responsibilities

• Perform comprehensive patient assessments and develop treatment plans
• Provide preventive care and manage acute and chronic conditions
• Educate patients and families to support informed health decisions
• Collaborate with physicians, nurses, and support staff
• Maintain accurate documentation and ensure quality standards

Qualifications

• Master's Degree from an accredited NP program
• Active Vermont APRN License (or ability to obtain)
• National NP Certification (Adult, Family, or Pediatric preferred)
• DEA License & CPR Certification
