POSITION SUMMARY:

THE CLINICAL TRIAGE RN (OR NP) ROLE IS AN EXCITING NEW OPPORTUNITY WITHIN CCA TO WORK ON OUR MOBILE INTEGRATED HEALTH PROGRAM. THIS RN OR NP WILL WORK FROM NOON UNTIL 8PM DOING COMPLEX TRIAGING AND COORDINATING OF OUR PROGRAM ALONG WITH OUR PARTNER, EASCARE.

Commonwealth Care Alliance s (CCA) Clinical Triage Registered Nurse is primarily responsible for receiving incoming calls from CCA members, patients, medical providers and others related to clinical conditions or concerns, including medical problems, behavioral health needs, clinical concerns and administrative questions. When appropriate, the nurse will use clinical triage protocols to assess the health condition of the caller to determine the most appropriate course of action based on assessment tools. The nurse will also assess clinical situations to determine the priority for action. When not responding to incoming calls, the nurse will outreach to members and patients following service provision to determine their status and satisfaction.

The Clinical Triage Registered Nurse reports to the Manager, Clinical Response.

RESPONSIBILITIES:

* Engages callers in a sincere, confident, and caring manner, instilling confidence in his/her ability to serve caller s needs

* Gathers pertinent demographic information to identify the caller.

* For clinical calls, utilizes clinical triage tools to ask standardized appropriate medical questions to help determine the complexity and urgency of the caller s condition.

* Determines the most appropriate course of action to serve the member s or patient s needs.

* Reviews requests for services that may be received electronically, determining if additional information is required.

* Prioritizes all service delivery based on acuity and urgency.

* Diffuses any caller anxiety and handle the situation with sensitivity and empathy.

* Communicates with the member/ patient about the service delivery, including recommended approach (telephonic, telehealth, mobile visit, ED visit) timeline for service, and advice for immediate care.

* Collaborates with community-based primary care providers to facilitate care coordination for new and existing CCA members


This role can also be done by a Nurse Practitioner, if so these additional tasks will included in the responsibilities:

* If incumbent is a Nurse Practitioner, telephonically provides medical direction to advanced practice paramedics who may be providing acute care in the member s/patient s home.

* If incumbent is a Nurse Practitioner, when appropriate, provides telephonic outreach calls to members/patients


MINIMUM QUALIFICATIONS:

* Registered Nurse with license in good standing in Massachusetts, required.

* Nurse Practitioner with licensure in good standing in Massachusetts preferred, but not required.

* 5+ years of meaningful clinical experience in primary care, inpatient, or urgent care setting, required.

* Previous telephonic triage experience or Emergency Room experience preferred

* Excellent communication and presentation skills.

* Effectively communicates complex and sensitive information.

* Displays strong customer orientation and ability to adapt/respond to different types of individuals.

* Able to troubleshoot: Curiosity to drive to get to the root of the problem.

* Confidence to make sound judgments.

* In-depth knowledge of available resources, clinical guidelines, systems and processes.

* Demonstrates initiative and is proactive with problem solving requiring minimal supervision.

* Broad comprehensive understanding of medical and behavioral diagnoses, social complexities and determinants of health.

* Exceptional knowledge of community based health care resources available in Massachusetts
