JOBS SUMMARY: Responsible for outreach, assessment, case management, support services, crisis response and facilitating groups with fathers. Work in coordination with other agency and community organizations and document services as required by program policies and funding sources. RESPONSIBILITIES: 1. Provision of direct services to fathers whose children are receiving DCF services: Outreach and engagement Assessment Formulation and development of a case management plan One-on-one case management Group facilitation utilizing the 24/7 Dad curriculum Community outreach and partnership development Assist fathers with navigating service systems, attend meetings and transport as needed Provide rotating on-call crisis response 2. Provision of Service in the community including home visits. 3. Accurate and timely completion of documentation and reports required by agency and program policies and funding sources. 4. Develop and maintain collaborative relationships with other community agencies and organizations in consultation with supervisor, in order to provide comprehensive and effective treatment services for clients (i.e. schools, courts, hospitals, etc.). 5. Work as a team member with FES Specialist and FES Supervisor to respond to client and programmatic needs. 6. Participate in all training, agency meetings, and funder requirements. 7. Knowledge and implementation of agency policies and procedures. 8. Partners with supervisor to identify areas and develop plans for professional growth. 9. Contributes to agency organizational life by participating in agency wide task forces, committees or problem solving teams as appropriate. 10. Contributes to agency visibility and development of community linkages by representing the Village in community groups or committees, as assigned by supervisor. 11. Other duties or responsibilities as determined by management. Experience and Skills: MINIMUM QUALIFICATIONS: Bachelors degree in social work, psychology, counseling or a closely related field; 3 years of experience in a comparable position in the human services field. Excellent oral and written skills and ability to communicate comfortably with fathers, family members, referral sources, and stakeholders. Organized and able to work independently with minimal supervision. Flexible and able to multi-task. Cultural competency to serve targeted populations. Bilingual skills preferred. Commitment to Results/Performance Based Accountability. Required to maintain valid drivers license and ability to drive or otherwise obtain transportation to provide services throughout the service area. Proficient with word processing and spreadsheet applications. Possess solid general computer skills, including ability to work in a Windows XP environment. Familiarity with specific applications used by The Village a plus. Physically able to perform the essential functions of the position, with or without reasonable accommodations; able to function safely in a therapeutic situations including provision of services in clients homes or host settings; able to travel to various sites throughout greater Hartford.
