Job Description:

SUMMARY:To provide an environment in which all activities, programs and plans recognize and implement strategies of community participation, relationship building/decision making, developing competency, and respect and dignity for each individual served.JOB SPECIFIC RESPONSIBILITIES:

• Actively seeks family and individual guidance and input for programming and direction. -
• Provides instruction and teaching strategies that include the use of generic and age appropriate materials.
• Ensures that activities are focused on each individual's capabilities.
• Implements individualized programs as agreed upon by consumer, family and coordinator.
• Completes necessary documentation and paperwork including visit logs, time sheets, travel, etc. according to specification and company guidelines.
• Meets regularly with supervisor to revise/update family needs.MCS WIDE RESPONSIBILITIES AND STANDARDS:
• Evidences in a positive manner the importance of MCS vision and values and mission statement as they relate to consumers, families, and staff.
• Exhibits a positive ethic, thus enabling the agency to render care in the most professional manner.
• Acts in a manner consistent with reporting to work as scheduled.
• Adheres to all safety regulations and policies.
• Meets MCS/DMR mandated core training.
• Attends staff meetings regularly and specific training related to individual consumer's needs.
• Has reliable transportation.QUALIFICATIONS:BS/BA preferred; one year experience in Human Service preferred; bilingual/bicultural for certain positions
