Housing Navigator, Friends of the Homeless

Job Description:

The Housing Navigator will be a key member of the Coordinated Entry System (CES) team that connects homeless individuals with services and housing. The Navigator will be assigned homeless individuals identified as a high priority within the CES that serves Hampden County. The Navigator will work with individuals to collect all documents needed to be placed in housing and will enter information into a shared data platform. The Navigator will provide additional support after a housing match is made to finalize the housing placement. This Navigator's position will be mobile and will meet with homeless individuals in the community approximately 75% of the time (with mileage reimbursement) and attend various meetings on site 25% of the time. Responsibilities: Work with homeless individuals identified as a high priority within the CES to get them housing ready such as documents, ID and income verification and assist with housing search and placement. Access the data system daily to obtain and manage caseload and keep up-to-date client records. Assist individuals that are homeless in gathering all documents necessary to obtain permanent supportive or other housing. Complete in-depth assessments as needed with clients to ensure proper supportive services are provided. Participate in and prepare for weekly meetings including staff meetings, Case Conferencing and other related meetings and provide regular feedback to CES Manager. Collaborate with a network of CES community partners including outreach workers, shelter case managers, housing and other service providers, and understand all roles and responsibilities within the CES. Be familiar with overall performance goals of CES and the Housing Navigator role and how to fully utilize the CES. QUALIFICATIONS: Bachelor's degree in social work or related field or relevant experience Required. Two years' work experience in housing search and/or case management. Demonstrated ability to work independently and as part of a team. Strong written and verbal communication skills. Problem solving and conflict resolution skills. Computer skills with proficiency in Microsoft office programs. Ability to work with diverse population and exercise good judgment. Ability to exercise good judgement and discretion in handling client confidentiality and other confidential matters. Ability to work both independently and collaboratively. Ability to contribute to and exemplify a collaborative and respectful environment that promotes teamwork. Knowledge of local social service and other community resources. Familiarity with Hampden County Continuum of Care. Working knowledge and experience with HMIS and VI-SPDAT preferred. Bilingual skills (English-Spanish). Must have strong organizational and time management skills. Self-motivated. Service oriented. Must have reliable personal transportation, a valid MA driver's license, auto insurance and an acceptable driving record.
