Job Description: ABOUT WOMEN'S REACH The Village is very excited to announce the start of a new, innovative team of recovery specialists working with pregnant and post-partum women who are struggling with addiction or substance use issues. The Womens REACH program will provide individual and group coaching services, recovery support services, treatment referrals, and assistance with connection to community services of all kinds. The program is based on the knowledge that recovery requires attention to the whole person. ABOUT THE VILLAGE The Village was one of the first agencies in the country to provide homes for neglected children. Today, we continue to achieve our mission 'to build a community of strong, healthy families who protect and nurture children' by providing a full range of behavioral health, early childhood and youth development, substance abuse treatment, and support services for children, families and adults in the Greater Hartford region. JOB SUMMARY: The Womens Recovery Navigator (WRN) works with pregnant and post-partum women who are at risk for or who have substance use disorders. The WRN offers recovery support and help with obtaining community-based services and resources, while providing hope and positive role modeling. She provides education, advocacy, coaching and mentorship to individuals choosing to work with a navigator. The Womens Recovery Navigator must be a woman with the lived experience of being a woman in recovery from a substance use or co-occurring disorder; she must be comfortable using her lived experience to help others. Through knowledge of the recovery community, including 12-step facilitation, and other community support resources, each navigator will help connect clients to services aimed at enhancing their recovery plan while supporting their goals as they develop a safe support network. The WRN is not intended to replace clinical services, but rather enhance them. If these services occur within a clinical setting, the WRN should act as a member of the clients clinical team. RESPONSIBILITIES: Responsibilities include, but are not limited to: Take steps to ensure client engagement from referral through transfer/discharge from service, including initial outreach to clients upon identification. Provide emotional support and connection to the women engaged in REACH. Provides education, advocacy, coaching and mentorship to individuals choosing to work with a navigator Provide Individual Coaching sessions to participants, initially offered weekly and then tapering the frequency as each client progresses through the program. Provide weekly recovery support groups in geographically convenient locations throughout DMHAS Region IV. Coordinate referrals to services, accompany women to scheduled appointments as needed, and role model self-advocacy skills to each participant. Participate in multidisciplinary team meetings with community and clinical teams throughout the region, as appropriate. Participate in educational programming and individual services to clients and families to provide information about recovery, addiction, use of Naloxone, and other relevant topics. Following Child Abuse Prevention and Treatment Act (CAPTA) legislative guidelines, work with the mothers to develop and implement a Plan of Safe Care for infants who are exposed to substance use during pregnancy. Serve as a resource for case management services to clients and their families. Assist clients in connecting with and gaining eligibility for entitlement programs, including public and/or private insurance programs. Participates in region-wide education and communications to hospitals, OB-GYN practices, birthing centers, and other appropriate referral sources about the REACH program, including education about warning/risk signs for substance use disorders, referral procedures, and community interventions. Develop and maintain collaborative relationships with other community agencies, organizations and referral sources in consultation with supervisor to provide comprehensive and effective treatment referrals for clients (i.e. schools, courts, hospitals, etc.) Maintain effective, positive working relationships with referral sources, including DMHAS, DOC and CSSD. Document all recovery activities according to the same principles used for clinical services at The Village. Meet all standards for timeliness and accuracy in performing data and reporting functions, including communication with referral sources. Produce and communicate timely and accurate progress reports, court reports and other documents, as appropriate. Comply with all agency policies and procedures, HIPAA regulations, Joint Commission standards and funding sources requirements. Participate in individual/group/team supervision, clinical quality improvement and Quality Assurance activities; participates in Utilization Review as requested by supervisor. Contributes to agency organizational life by participating in agency wide task forces, committees or problem solving teams, as requested. Contributes to agency visibility and development of community linkages by representing the Village in community groups or committees, when directed by supervisor. Other duties and responsibilities as assigned by management. Experience and Skills: Must be a self-identified woman in recovery. Must have a minimum of two years of sobriety. Must have completed the Connecticut Community for Addiction Recoverys (CCAR) Recovery Coach Academy, or must complete that program within 2 months of hire. (Documentation of successful completion will be required). Must demonstrate a wide knowledge of community-based recovery and treatment services in DMHAS Region IV. Must be able to communicate comfortably with clients, family members and significant others, and community resources and professionals about recovery-oriented topics, including the RCs personal recovery. Must be organized and able to work independently with minimal supervision. Should be flexible and able to multi-task. Must be able to use relevant computer software, including word processing electronic health records. Ability to learn and work with specific applications used by The Village and its referral sources is necessary. Must be culturally competent to serve diverse client population; bi-lingual (English/Spanish) strongly preferred. Physically able to perform the essential functions of the position, with or without reasonable accommodations, i.e. must be able to supervise groups of children and perform physical intervention if required; must be able to move freely from place to place without restriction.
