please visit our website at www.refugees.org. OVERVIEW The Refugee
Health & Wellness Program focused on support, advocacy, and case
management services for refugees with chronic or critical medical
or mental health issues. The Refugee Health Promotion (RHP) case
manager will provide health education, healthcare system
orientation, and case management to newly arrived refugees in
Albany and the Capital Region. This position reports to the Health
& Wellness Manager and the Albany Field Office Director. ESSENTIAL
DUTIES AND RESPONSIBILITIES Provide direct case management services
to refugees in the community under supervision of Health & Wellness
Program staff. Conduct home visits, intakes, and assessments of
clients referred to the Health & Wellness Program under supervision
of Wellness Program staff. Coordinate appointments and services,
and maintain case files and case notes for clients enrolled in the
Wellness Program. Communicate with healthcare providers about
ongoing barriers or needs of clients served in the Wellness
Program, and work to eliminate these barriers. Provide education
and orientation to small groups of refugees to support their
healthcare literacy. Work to connect clients served in the Wellness
Program with supportive community networks and social support
systems to ensure their long-term integration into the community
and well-being. Perform other job-related duties as assigned by
supervisor. All of the above duties and responsibilities are

supervisor(s). POSITION REQUIREMENTS Bachelor's degree in social
work, counseling, public health, psychology, or a relevant field
preferred. Prior experience providing case management services to
refugees, asylees, and other immigrants preferred. Excellent
interpersonal skills, including ability to listen, communicate
clearly, and build relationships and partnerships with program and
Field Office staff. Excellent organizational, verbal, written, and
interpersonal communication skills. Proficient in Microsoft Office,
specifically Word, Access, and Excel and other database programs is
required. Highly adaptable and flexible with the ability to
multi-task and prioritize in a demanding work environment. Must
have a valid state-issued driver's license, automobile insurance,
clean driving record and reliable personal transportation. Must
pass a criminal background and other screening requirements.
Ability to travel locally as required. Demonstrated support of

vaccination card mandatory. TRAINING REQUIREMENTS Satisfactory
completion of USCRI’s Orientation and Training. Complete additional
training as identified by supervisor or Human Resources PHYSICAL
DEMANDS Use of manual dexterity, tactile, visual, and audio acuity.
Use of repetitive motion, prolonged periods of sitting and
standing, and sustained visual and mental applications and demands.
Occasional lifting (up to 25 pounds), bending, pulling, and
carrying. Reasonable accommodations may be made to enable
